admission

The day of your arrival to Najjar Hospital, you will be greeted by the Admission officer.

You will need:

• Your personal identification card

• Your admission slip filled by the admitting physician

• Insurance card or prior authorization form from the guarantor for non-self payers

• Signed consent form (patient authorization for a service or procedure)

After being admitted to your room, you will receive an identification bracelet with your name and reference number. Please keep the bracelet on until you have been discharged from the hospital.
